Coach Agreement Form

(1 needed per school per season) – this is a digital Google Form and comes directly to NCSO staff. Schools qualifying for states do NOT need to submit this form again.

Student Waiver / Photo Consent Form

The Student Waiver/Photo Consent Form must be completed by a parent BEFORE competition day. The parent will receive a confirmation email that they should forward to the head coach as a receipt that the form is complete. If the form is not completed by a parent the student is NOT permitted to compete.

It is the coaches responsibility to make sure all waiver/photo consent forms are complete before tournament day.
