Coach Agreement Form

(1 needed per school per season) – this is a digital Google Form and comes directly to NCSO staff. Schools qualifying for states do NOT need to submit this form again.

Team Photo Consent Form

(1 needed per team per tournament) – this is a hard copy form that must be turned in to your Regional Director at Coach Registration the day of the tournament. This form must be completed again if you qualify for states.
