Coach Agreement

This form must be completed by each Head Coach (one per school) before competition. This form will be kept on file for the duration of the tournament season and only needs to be completed once per year. Please communicate any changes or updates to the Head Coach role to the NCSO Director Lisa Giacomelli ([email protected]) throughout the season, if needed.

Parental Consent/Student Waiver Form

The Student Waiver/Photo Consent Form must be completed by a parent BEFORE competition day. The parent will receive a confirmation email that they should forward to the head coach as a receipt that the form is complete. If the form is not completed by a parent the student is NOT permitted to compete.

It is the coaches responsibility to make sure all waiver/photo consent forms are complete before tournament day.
